The City of Johnson City Offers Each Employee and Their Spouses an Annual Wellness Screening!

to you, the completion of the screening process by December 31, 2021, will also result in a lower insurance premium.
FIRST: Enter your DOB and the last four of your SSN below. Choose from the pulldown menu what population group you belong to:
- active employee
- spouse
- retiree
- retiree spouse
NEXT: Once your survey is complete, you will be directed to schedule an Assessment appointment or a Biometric Lab Review appointment.
Assessment appointments
You will schedule an Assessment appointment if you intend to have your labs drawn at your assessment. The Assessment will include a blood draw and a basic biometric screening including: resting blood pressure, heart rate, height, weight, and calculation of body mass index. Please drink plenty of water to keep yourself hydrated. This makes obtaining labs much easier! The Assessment appointment will be held at the Business Health office at 403 Princeton Rd., Suite 3. Located in the same building as the EHC.
You will need to be fasting for all labs drawn whether they are performed at your Provider’s office or during the Assessment appointment.
Fasting means: no food, no drink other than plain water or black coffee (no sugar, no cream or anything added), no chewing gum, no mints/candy and no use of smokeless tobacco.
Biometric Lab Review appointments
You will schedule a Biometric Lab Review appointment if you intend to bring your own labs from Primary Care Provider. Your lab results must be dated within 90 days of your appointment date and include: a fasting glucose and a lipid panel (cholesterol, LDL, HDL, triglycerides).The appointment will consist of a biometric screening and review of your labs. These appointments will be conducted at the Employee Health Center, at 403 Princeton Rd, Suite 10 with the Nurse and will include the basic biometric screening as well as a consult.
Consult Appointment
If you complete an Assessment appointment you will also need to complete a Consult with the Wellness nurse. They will review your labs and discuss the results of your biometric testing. The Consult appointment will be scheduled at the time of your Assessment appointment.
